Bloom offers eclectic and comprehensive treatment services specific to mental health and addiction on your time. Bloom assists clients through in person or a virtual telehealth platform to meet with qualified psychiatrists, medical providers, skilled and licensed therapist, to meet all their treatment needs.

Bloom is in-network with most major insurance carriers including Highmark, UPMC, Aetna, Cigna/Evernorth, Allegheny County Medicaid (CCBH) and offers a flexible fee-for-service model to accommodate the financial needs of our clients.
Staff at bloom including psychiatrist, certified registered nurse practitioners, therapists, and physician’s assistants are here to aid in your mental health conditions. They treat a wide array of mental health conditions such as depression, anxiety, post-traumatic stress disorder, attention deficit hyperactivity disorder, obsessive-compulsive disorder, and other behavioral health conditions. Meet with one of our skilled prescribers to assess your specific condition and formulate a plan that works for you.
All Cancelations must be made 48 hours in advance. Any appointment cancelled under 48 hours will result in a $60 late cancellation fee. If you do not attend a scheduled appointment and fail to cancel in advance, you will be charged a no-show fee based on the appointment type:
* Please note: These fees do not apply to individuals covered by Medicaid, in accordance with state and federal guidelines.

All medication refill and prior auth request must be completed in our online form portal.
Please be advised that any refill request completed as a result of a missed appointment or lost medication there will be an administrative fee of $25. To avoid this fee you can schedule with your provider and address the refill at the time of your next appointment.
Repeated medication refill requests will not be authorized. Additionally, patients who have not been seen in the office for a long time or who frequently miss their appointments may not be provided with refills.
Fill in the form completely including the accurate pharmacy information. It may take up to 72 business hours to complete a refill request/prior authorization or longer if during weekends and holidays. Please do not call the office to check on the status of your request.
Please call your pharmacy to see if they’ve received your prescription. You should have more than 3 days of medication left at home when you request a refill.
Medication Refills will not be authorized under the following situations:
* You do not currently have a follow up visit scheduled with one of our providers
*You have not been seen for visit by your provider in the past six months.
*You have submitted more than 2 refill requests in the past 365 days.
*You have an outstanding balance.
